MDE 2019 Auction Donation Form

Donations forms are due Friday, January 11, 2019. Please complete this form for the MDE Auction Chairs and Auction Catalog. Donations are tax deductible to the extent provided for by law. Memorial Drive Elementary PTA is a 501 (c)(3) tax exempt organization (Tax ID 76-0681143). Upon submitting this form, you will receive a confirmation email that will serve as your receipt. Please retain a copy for your records and tax purposes. No goods or services were received in exchange for this donation.

  • Please select your preference of delivery for your item.
  • Please let us know if there are restricted dates, expirations, etc. for your donation. Keep in mind the auction date is February 9, 2019 when setting expiration date.
